The Indiana Durable Power of Attorney (Financial) form allows you to set a specific person to divvy up your special belongings, money and take care of your finances when you are no longer able to. This Power of Attorney will administer your personal wishes you declare in this document.

A Power of Attorney allows someone to act in place of another person. In the case of this form, the agent will take authority over your financial life. After taking time carefully choosing your agent, you can list the specific responsibilities he or she will have. The agent's responsibilities are very important. This person will have control over your finances, when you can no longer speak for yourself, so choose wisely. You can choose for your agent to be responsible for all the powers in this document or only a select few. These responsibilities range from collecting government benefits, managing taxes, insurance and real estate, to providing gifts to family or friends. If there is something that wasn’t listed that you would like to add, you can also add special instructions.

You may print this form out to complete filling it in and signing it. From there, you can upload it right back into the system to your ezEstatePlanner Vault.
