District of Columbia Living Will ( Declaration)

Last Updated: 11/23/2015

Document Description

Someone once said, the only things certain in life are taxes and death. While neither are fun to think about, one must be prepared. The District of Columbia Declaration, may not help with your taxes, but it will help prepare for end of life care. More commonly known as an advanced health directive or living will, this Declaration, helps you decide your options regarding health care, for when you are incapacitated due to terminal illness or a persistent vegetative state.

A living will is there to speak for you when you are no longer able to speak for yourself. It helps your loved ones make tough decisions about your care towards the end of your life. In your declaration, you can state your decisions regarding medical treatments that include artificially prolonging your life with methods that can include respirators, feeding tubes, and medications. You are also able to determine where you would like to receive treatment in your home, a hospital, or hospice. Your District of Columbia Declaration, will only take effect once you become incapacitated due to terminal illness or persistent vegetative state. These conditions must be certified by two physicians prior to the Declaration to take effect.

Things change. We get it. You are able to make changes to your District of Columbia Declaration. This living will can be adjusted because of changes in medical advancements, marriage, divorce, or just your personal beliefs. When and if you decide to make changes to your living will, you can do so by voiding the old one, and creating a new living will on ezEstatePlanner. It is imperative that your living will is accessible to those closest to you when the need arises. It should be given to your immediate family, loved ones, and medical professionals. You may also store your DC Declaration in your ezEstatePlanner Vault. The vault can also be used to store cherished photos, and other estate planning documents like, power of attorneys or life insurance documents.
