Document Description
Making a decision about whether or not to prolong a loved one’s life is not easy. It can cause disagreements among family members and make the mourning process even more difficult. These decisions may be very tough, but making them ahead of time can be extremely beneficial for you and your loved ones.
The Illinois Advance Health Directive, also known as a declaration or living will, allows you to state to medical officials your specific choices regarding end-of-life treatments. This form is only used at the end of your life, whether caused by disease, old age or serious injury. Whether you are in a persistent vegetative state or have a terminal condition, the Illinois Advance Health Directive allows you to make your decisions regarding care ahead of time . Would you like life-sustaining treatments to prolong your life even though you are incurable? You can choose such an option in this living will.
Some of the many decisions you have to make include feeding tubes and organ donation. Another important factor is where you want your care. At the end of your life, would you want to be at home with your family with nurses or medical care, or would you prefer the hospital or hospice? Creating this form can make a difficult time much less burdensome.
Add this form to your ezEstatePlanner Vault to be retrieved if ever needed. You can also include your will, power of attorney forms or any other estate planning documents you may have created. The Vault can also store other important files you may need at the end of your life. These documents may be retrieved by your keeper, a person you designate to have access using a separate log-in. This person can be a loved one, an attorney or whomever you see fit.
