Life moves fast, sometimes faster than we expect. While we can never be fully prepared for what life throws at us, we can try. One tool is the ezEstatePlanner Maryland Advance Health Directive. This helps plan for when life is coming to an end. Also known as a living will, an advance health directive is a set of written instructions that tells your family, medical professionals and loved ones your end-of-life plans and decisions.

The ezEstatePlanner Maryland Advance Health Directive ensures your final requests are carried out. It gives you the ability to communicate with medical professionals when you are no longer able and helps to make sure your personal and religious beliefs are respected. This legally binding form lets you choose options regarding your quality of life when you can no longer speak for yourself, including pain medication, feeding tubes and your preference for hospitals, hospice or home care. You are also able to note additional directions regarding who can make decisions on your behalf and what should be done with your remains.

The Maryland Advance Health Directive is not valid in the event of medical emergencies, as ambulance and ER personnel are required to provide CPR. If you do not want resuscitation, a separate DNR (Do Not Resuscitate) form is required. It is important to note that the Maryland Advance Health Directive also does not address mental illness. To create an Advance Health Directive in the case of mental illness, you will need a Maryland Advance Directive for Mental Health.

It is important that this document is accessible to those who will need it. Copies should be given to family and/or close friends, as well as medical professionals. You are also able to store your signed Maryland Advance Health Directive along with your other estate planning documents in the ezEstatePlanner Vault.
