These questions may be hard to think about, let alone answer, but they are necessary to make sure your final wishes are honored. The Massachusetts Advance Health Directive helps plan for the future. With this document, you are able to plan how you will spend the final days of your life.

An advance health directive is a statement of personal decisions regarding your health for when you are no longer able to make decisions on your own. This form, which is also known as a living will, lists your choices for life-sustaining treatments should the need arise. You are also able to choose whether you would like to have feeding tubes to sustain nutrition or pain medication to keep you comfortable. With the Massachusetts Advance Health Directive, you can decide if you would like to be placed in a hospital, in hospice care or if you would like to spend your last days at home.

Currently, the state of Massachusetts is one of three states that does not recognize living wills, but it is useful to have an advance health directive to make sure your wishes are known. In your Massachusetts Advance Health Directive, you are able to appoint an agent who can make decisions on your behalf, if an unforeseen circumstance arises. Your desires should be made known to those who need this information most, including your medical professionals.

A copy of your Massachusetts Advance Health Directive should be given to close family and friends, as well as medical professionals, so the information can be readily available in case of emergency. You can also upload a signed copy along with your other estate planning documents, files and photos to your ezEstatePlanner Vault.
