Have you ever thought about how or where you want to be for your last moments? Some people choose to be in the comfort of their homes surrounded by loved ones, and others choose to be closely watched in a medical facility. This decision and many others can be made with an advance health directive.

The Michigan Document Directing Health Care, also known as a living will or an advance health directive, is the proper form used to make your wishes for the end of your life. Although this may be a difficult subject to think about ahead of time, it is important to do so. Leaving these decisions for family members can make the grieving process more difficult or stir up disagreements among family members.

Instead of having someone else make the decisions for you, declare your end-of-life preferences in this form. The living will allows you to choose whether you would like to have home care, hospice or hospital care and is only used when you are at the end of your life. You will need to decide whether you would like healthcare workers to give you life-sustaining treatments to prolong your life if you have a terminal condition or are in a vegetative state. You can also include your preferences regarding feeding tubes and other additional directions.

After filling out the Michigan Document Directing Health Care, signing it and having witnesses sign it, you can upload it right back into your ezEstatePlanner Vault. There, you can keep all of your personal files that you would like retrieved at the end of your life.
