An advance health directive allows you to make decisions regarding your health care when you are no longer able to. This set of instructions, also called a living will, includes choices concerning end-of-life care such as pain medication, life-sustaining treatments and feeding tubes.

Advance health directives are not required but are a good way to prepare for the end of your life. The plans you make now will help assist your loved ones in making decisions in the future. A living will is a great resource for planning how to spend your last days and is a way to communicate with medical professionals, family and loved ones. Having this document ensures that your medical wishes are carried out during a time when you may not be able to convey them.

The Missouri Advance Health Directive enables you to add choices for health care once you become incapacitated. You can decide beforehand what life-sustaining treatments you would like and which you prefer not to receive. You may also state where you want to spend your last days, whether in a hospital, hospice or home. Please note that an advance health directive is not a permanent document. If you change your mind in the future about treatment options or proxies, you are able to create an updated directive.

It is a great idea to print out your completed and signed advance health directive. This should be given to close friends and family members in case of emergency.
