The New Hampshire Advanced Health Directive allows you to implement plans to ensure your wishes regarding end-of-life care are honored in the event you are no longer able to make health-care decisions due to a terminal illness or injury or if you are near death. These directions help loved ones know how to care for you during this difficult time.

An advance health directive, or living will, is a set of written instructions that spells out your directions for health care when you are no longer able to make decisions on your own. While it is a great idea to have a living will on file, sometimes our views or desires may change based on medical breakthroughs, personal beliefs or an epiphany. No matter the reason, your New Hampshire Advanced Directive can be changed and updated.

A living will does determine end-of-life care based on your preferences. However, it does not affect any emergency treatment such as CPR. If you would like to avoid any resuscitation, a New Hampshire Do Not Resuscitate order must be completed.

It is a good idea to keep your advance health directive, along with any DNR orders or other estate planning forms, in your ezEstatePlanner Vault. Here you can upload completed and signed documents to store for future use. If for any reason you become incapacitated, the Keeper whom you assign will have access to your forms so your wishes can be carried out. You should also make sure your loved ones, family and medical professionals have a copy of this document on file.

In order to be valid, the New Hampshire Advance Health Directive must be signed by two witnesses and a notary public. Power only begins once you are no longer able to make decisions regarding your own health care, which must be certified by two physicians.

Keep in mind that the New Hampshire Advance Health Directive is valid only in New Hampshire. If you visit multiple states often, it is a good idea to create living wills for each of those states as well.
