Each one of us will be required to make an important health care decision at some point in our life. In this situation, we must provide consent to our doctors, allowing them to treat us as per our wishes. But what happens in a situation where we are unable to speak for ourselves?

A living will can speak for us when we are not able to. Also called an advance health directive, a living will is a legally binding document. It provides guidance for our medical professionals and loved ones by explaining our wishes about different treatments and medical procedures. It is used only when we are in facing a serious or terminal condition and cannot speak for ourselves.

If you would like to be in charge of your own health care decisions, complete our living will form. Our form is written exclusively for residents of Ohio, as it follows Ohio state laws and regulations. Once completed, signed, dated and witnessed, your living will becomes a legal document.
