Pennsylvania Living Will (Advance Health Directive)

Last Updated: 11/23/2015

Document Description

There’s no denying it - every day you get older and there is no way to pause time or stop it. Before reaching old age, it is critical to have all of your legal ducks in a row. In the event that you become unconscious, incompetent or have a terminal condition, it is important to have your medical decisions prepared beforehand. The Pennsylvania Advance Health Directive, also referred to as a living will, allows you to express your health care decisions for when that time comes. If you are unable to communicate what kind of treatment you would prefer, preparing this document in advance allows you to convey your wishes.

There are many choices that come into play when figuring out your end-of-life preferences. Whether you would like to be at your home, in hospice care or at the hospital, it is important for you to choose with what you are the most comfortable or what the best option is for you. The Pennsylvania Living Will allows you to choose whether or not you would like life-sustaining treatments that may or may not prolong your life. The option of feeding tubes is also one of the many choices you should consider while creating this form.

It is important to be of sound mind and in a sane state while creating the advance health directive. You may sit down with your family and discuss these options, or choose them by yourself.

After taking time to complete the Pennsylvania Advance Health Directive, you can add it in your ezEstatePlanner Vault.
