Anything can happen. Whether you are going in for a big surgery or are in perfect health, it is extremely important to have the proper documentation for any possibilities that could occur. An advance health directive, also referred to as a living will, gives direction to an agent, family member or health care professionals. The document lists your options regarding any medical choices that you would need to make and will be used if you no longer can make decisions for yourself.

It is important to create this document while of sound mind. Making life decisions for a loved one can never be too easy, so make sure you take the time to choose the right person for this job. When filling out this form, you can include any specifics you deem critical.

The Utah Advance Healthcare Directive allows you the power to select your preferences regarding the actions you want physicians, doctors or health care professionals to handle in case you become terminally ill or are in a persistent vegetative state. Including whether or not you want your life prolonged by life-sustaining procedures is a key decision in your end-of-life options.

You may also indicate your choice between hospital care, hospice or home care. If you would like to include any further verbiage in relation to your medical preferences, feel free to add that in section IV. You may also attach an Organ Donation Statement to your Utah Advance Healthcare Directive. Please note that this document has no effect if you are pregnant.
