We all have decisions to make, some that may be a little harder or more uncomfortable than others. However, making these decisions ahead of time, along with proper planning, can significantly improve those awkward moments.

The Vermont Advance Health Directive, also referred to as a living will, allows you to prepare, direct and specify your choices regarding your end-of-life care. These decisions may be difficult for family members to make, and sometimes they do not know what decisions you would prefer. This document will allow you to include all of your special terms in the event you have a terminal condition or are in a persistent vegetative state. Some preferences you will choose include life-sustaining treatments, where you want your last days spent, organ donation and even feeding tubes.

Preparing this form ahead of time will take the responsibility off of your loved ones. The Vermont Advance Health Directive has everything laid out to give directly to healthcare officials in the case of a serious medical issue. Creating this form allows them to follow your specific end-of-life wishes.

After you create your living will, you can have two witnesses sign it. Once you have it filled out to your liking, you can add it right back onto the website in the ezEstatePlanner Vault.
