An advance health directive, also known as a living will, records your wishes about medical care in the event that you are incapacitated. Do you want to be kept alive on life support if you slip into a persistent vegetative state? Do you want hospital care, or would you rather have hospice care at home?

These decisions may be difficult, but they are far easier for you to make about yourself than for your family to have to make on your behalf, should something happen to you. Family members should never have to decide whether to install feeding tubes, or maintain life support, especially while already grieving that something has happened to you. It's critical that everyone make these decisions for themselves, while they are healthy, and record these wishes legally.

Our New York Document Directing Health Care (or living will) complies with all New York laws and regulations, and includes all necessary language for New York.

Once you have drafted your New York living will, and have it signed and witnessed, you can store it in your secure Estate Vault. Aside from being a secure place to store your New York health directive along with your last will and other estate planning documents, you can also share access to individual documents with anyone you choose. In the event you are incapacitated, they will be able to access your living will and other critical documents (after verifying their identity).

Note: A living will/advance health directive is related but different from a medical power of attorney. While a health directive states your wishes for your health care, a medical power of attorney assigns responsibility and power to another person to make health care decisions on your behalf if you are incapacitated.
