Want to cancel a previous health care power of attorney? No problem.
This Revocation of Durable Medical Power of Attorney will void, cancel, nullify, retract and otherwise render obsolete the previous medical power of attorney document that you signed.
Everyone makes mistakes or changes their mind sometimes, so this document will let you revoke your prior power of attorney, and (if you so choose) draw up and sign a new health care power of attorney. It's important that you notify both the old agent (from the revoked power of attorney) and the new agent (if you have signed a new document), as both need to be aware of the change in their responsibilities.
When having this document witnessed, be sure to choose witnesses with no interest in either the old or the new medical power of attorney, to avoid disputes later. While a notary's seal and signature are generally not required, they certainly don't hurt in verifying your intent and the document's authenticity.
As a final note, it's important to create or revoke medical power of attorney while healthy and of lucid mind. Others may dispute your mental capacity later, if you wait until after a health care power of attorney becomes necessary.
